Keisha Nash’s cause of death has been determined.
The certificate of death notes that Nash had lived with the disease for “years” and lists acute renal failure as another significant condition that may have contributed to her death. It also lists Nash’s daughter with Whitaker, True, as the informant of her mother’s death.
The former model was buried in a service in Los Angeles on Dec. 20, as the document reads.
A representative for Whitaker did not immediately respond to PEOPLE’s request for comment Tuesday.

At the time, True also shared a photo of her mother with lyrics from soul singer Donny Hathaway’s “A Song For You” playing over the picture.Deadlineconfirmed the news of Nash’s death with her family at the time.

Whitaker, 62, and Nash first met on the set of the 1994 filmBlown Awayafter she was cast to play his character’s girlfriend, perCBS News. Whitaker proposed the following year and the couple married in 1996 in Montego Bay, Jamaica. In addition to True, the pair also share daughter Sonnet, 27.
TheAcademy Awardwinner also has an older son named Ocean, 33, while Nash has Autumn, 32, both from previous relationships.
The late actress was also the creator of Kissable Couture, a luxury cosmetics line she started with makeup artist A.J. Crimson.
“I’ve always been very ambitious,”she toldO, The Oprah Magazinein 2008 about her work for the brand. “When my kids were younger, I tried to start a clothing line for children. But you have to be really committed to a new business, and at that time I was more focused on being a mother.”
